Forward Thinking Fitness | Terms of Service

Effective 01.01.2023

By rendering services in our programs, including the use of our proprietary technology, you as a client agree to the following:

1. Upon being accepted as a client in one of the programs at Forward Thinking Fitness, clients are responsible for 100% of installment dues, with no exception. Program duration is a range of 8 to 10 weeks, dictated by a scheduled/completed CheckPoint Session to scheduled/completed CheckPoint Session.

2. Scheduling cycles reset every Sunday at approximately 12:15 am ET. All services must be scheduled and rendered Monday through Saturday of the cycle. Clients are solely responsible for managing their appointments with no exceptions.

3. All non-refundable program dues are processed every Sunday at approximately 12:15 am via checking account (0.0% fee) and credit card (3.99% fee). Payment form may be switched securely at any time via phone call or in-person.

4. Services may be scheduled between 6 days and 6 hours in advance of a session start time. Clients may request help for appointments outside of the scheduling window, but with no guarantees of availability.

5. Appointments may be early-canceled in our mobile app 6+ hours in advance of session start time without penalty, without exception.

6. Appointments may be late-canceled in our mobile app less than 6 hours in advance of session start time. Sessions are forfeited as if attended and subjected to a $15 charitable-fee, without exception.

7. Appointments may be no-showed (not canceled by client in mobile app). Sessions are forfeited as if attended and subjected to a $20 charitable-fee, without exception.

8. Clients should plan to arrive 10 minutes early for a scheduled appointment to maintain the safety of our community. Tardiness (5 minutes or more) to a session is subjected to a $10 charitable-fee, without exception.

9. Customized combination of services may only be adjusted or terminated at each scheduled/ completed CheckPoint without exception. Heart rate monitor systems (all pieces) must be returned during this time to effectively terminate the account to avoid a $99 fee.

10. Scheduling cycles may be proactively suspended for medical procedures or illness/injury at no cost. Requested suspension dates with accompanied documentation from a healthcare

provider must be submitted to hello@trainftf.com no later than Friday at 1 pm prior to the next scheduling cycle without exception.

11. Heart rate monitor systems are maintained through replacement pieces by Forward Thinking Fitness with no additional charge. Lost or stolen systems may be replaced at cost ($99) without exception.

12. Clients are responsible for providing all updates on developing health issues prior to a session for our coaches to effectively adjust programming to a safe level.

13. All prescribed movement patterns are to be pain-free with no exception. Clients are responsible for communicating any pain experienced during a session for our coaches to adjust programming to a safe level.

14. Clients are responsible for checking in for their scheduled service through our mobile app or the provided kiosk located in the lobby. Failure to sign-in will result in either a late-show or no-show fee without exception.

15. In the event of inclement weather, all scheduling policies above are void. Virtual sessions are provided in lieu of on-site attendance for all offering all year round.

16. Two Shoe Rule is effective during the winter season (November-March) to prevent salt from tracking onto the training floors. Clients are required to change from street shoes to training shoes upon entering the lobby during this time.

17. Clients who join our Perks Program will be credited with points that can be redeemed for retail purchases, services, and exclusive offers. Points are cumulative during the life cycle of the client and expire within 30 days after account termination.

18. Clients are expected to be respectful and kind to all members of our community (clients and staff) at all times while maintaining nothing short of a positive attitude upon entering the facility without exception.

19. Forward Thinking Fitness reserves the right to adjust the service schedule and the right to terminate services for clients unwilling or unable to comply with all terms and conditions above.
